What Is the SMAS?

SMAS stands for Superficial Musculoaponeurotic System.

It is a fibromuscular layer located beneath the skin and subcutaneous fat of the face. Because the SMAS contributes to facial support, many modern facelift techniques involve manipulating this layer.

In an SMAS facelift, the surgeon may tighten, fold, reposition, or partially release the SMAS depending on the specific technique being used.

The skin can then be redraped with less reliance on skin tension alone.

What Is a Deep Plane Facelift?

A deep plane facelift involves surgical work beneath portions of the SMAS.

Instead of separating the skin extensively from the SMAS in certain areas, tissues may be elevated together as a deeper composite unit after selected facial retaining structures are released.

This can allow repositioning of descended tissues in parts of the cheek and lower face.

However, “deep plane” does not automatically mean that every structure is dissected more deeply or that the procedure is suitable for every patient.
